What I want to talk about today is, something we’ve kind of covered a little bit earlier, a couple of episodes ago about just the amazing shackles that insurance puts on you as a doctor or a provider where you can’t grow, you can’t become better and better and better until you’re the best in your field, because the insurance company won’t pay for what you’re trying to do. But Doctor Moore and I have had this incredible opportunity to grow from his knowledge of ultrasound, which has been awesome for me because it took a long time, ten years or more to get even, just basically good at it.
And he and I still get to work together all the time because I bring him into our office several times a year, to take care of very complicated cases and to continue to teach us so that we can bring the standard up higher and higher and higher. And we can do that because we are UNSHACKLED from the insurance world, because we’re not limited by what insurance covers.
In other words, we do stem cell therapies. We do PRP, we do regenerative medicine. We do all sorts of things that requires some really detailed ultrasound evaluation. And then, injection therapies with materials that are proven to be good but not covered by insurance. So, let me introduce you to doctor Randy Moore. Welcome aboard. I’m glad you’re here.
Dr. Moore: Thanks for having me.
Dr. J: Yeah, I’m glad you’re here in the office, this week. And we’re doing some great stuff with patients. We’re knocking it out where people who they don’t have a shot at getting a whole lot better with traditional medicine. We’re doing stem cell therapy on a patient we just saw. Who’s got a horrible neck situation. She’s got arthritis from the top of her neck, base of her skull, all the way down to her back, and she can’t move her neck. Well, she can’t move her head well, and there’s no option except to screw everything together in hardware brace and make it worse. Right. So how did we meet? Tell you how we met?
Dr. Moore: Well, you and I met several years ago, through, a colleague of mine up in Montana, Doctor Alexander. But anyway, we I was working up there in Montana at his clinic, and I was doing the regenerative medicine, ultrasound evaluation and ultrasound guided procedures. And then Dr. J, you came up, to check it out, to go see how you could get better at this type of, work.
But then also, you needed to be treated. And so.
Dr. J: That’s right. I forgot about that. Yeah. So much better.
Dr. Moore: Yeah, I forgot we treated your foot right,
Dr. J: And I have no problem with my foot now, by the way. Turf toe bad. Dr. Moore: You got a really good treatment on your foot, and you, remember you praised me right during the procedure about what a good nerve block it was.
So you can didn’t feel all those needles going in your foot, which is what had to happen. But anyway, so that’s how we met. We became friends, and you had to come down here to your office who began doing some work. And it’s just been a really great, fun relationship and helping a lot of people for quite a while.
Dr. J: Well, and what’s really neat about it is that, I was invited to be part of this, this training environment. Dr. Alexander is a researcher, and he’s just a really brilliant guy. He also does, oral surgery and all sorts of other things. He’s like an admiral in Navy or some kind of high ranking guy. He’s just amazing, man. And, he invited us to learn from him. All that he does is not covered by insurance. And what we have to do then is we have to go there, and I have to pay, a substantial amount of money to be trained because he’s got to take time out of his schedule to train me instead of making a living.
I can do that because I have a cash practice that makes enough money. I can afford to go get trained to be better. So there’s this virtuous cycle in cash practice. And I want to invite all you cash practice folks who are either in cash practice now or you’re contemplating could I do it?
Can I do it? If you don’t do it, you’re missing out on the opportunity to just become better and better and better and actually become, I hesitate to say it about myself, but you can become amazing at your craft because I have the advantage of working with people like Dr. Moore, who I can afford to pay to come down and we’re going to treat some patients together who are complicated, who are difficult, who I’ve learned to treat over these past 10 or 15 years through that kind of collegial work where I have to pay money for it.
The patients pay outside of the insurance model, and they get this care, and we’re building this entire community network and strategy for caring for patients in ways that the insurance company doesn’t cover. So if you’re in a cash practice, you can do that.
Dr. Moore: If you’re in an insurance practice you’re extremely limited.
You know, with your Cash Practice Solution program that you have here, Dr. J. I think a couple things come to mind is that, you know, when you become a physician, what you really want to do is you want to help people and you get yourself locked into an insurance-based practice. And they’re basically dictating to you what you can do, what you can’t do.
You’re out there, you need to run a practice. You to be profitable. I get that, but that’s not what you signed up for when you became a doctor, what you really signed up for was to help people. And doing a cash practice frees you up to provide that type of service.
And now if you combine going into cash practice, but then also bring in something like a regenerative medicine practice where it doesn’t have to be super deep, you can start off with doing platelet rich plasma, and I could help train you in how to do these basic protocols. It doesn’t have to be completely overwhelming.
Dr. J: That’s a shameless plug. That’s great. I love it, but. And the thing is, what? I want to communicate as well in the cash practice model. We are not competitors. We are colleagues. Right? Right. And the thing is, it’s a rising tide lifts all boats kind of thing if I help him be more successful and he helps me be more successful, I think that’s a win. Outside of that, in the insurance model you have people who are competing for insurance contracts. It’s ridiculous. It’s nauseating, I hate it. But in this model, I want to promote him. You should if you need training and you can get him to take the time because he’s super busy and he has about a minute every year to spare, to do these things. But he spares it for me and I’m thankful for that. And we take care of patients who are, they’re sick in ways that the typical care is not going to get them there. We provide them with the care that does, and then they go out and evangelize to the world that, hey, this guy doesn’t take insurance and, you know, it’s thousands of dollars to get the treatment.
But I’m better. And I had no other option.
Dr Moore: I think for people that are wanting to move into a cash practice, this is a really nice, segue for them to be getting offers, services like that. Because the public is aware of this. You go out and talk to friends around town and you say, I had stem cell therapy, I had platelet rich plasma or something like that. Oh yeah, I’ve heard about it, you know, so it’s not something that’s brand new. So to promote it as a cash practice, you’re not out there selling the latest fish oil or snake oil type of thing. You know, you have something that already has some public credibility, but there’s also a lot of research to back it up.
So you can bring this into your cash, practice and use J’s model for that, but then actually has an actual, you know, viable service that you would feel good about delivering, you know? So, I think that’s a great blend.

Dr. J: The reality is that in a cash practice, you have to provide people with amazing outcomes. If what you’re doing is providing people with a prescription like the regular doc down the street who takes insurance, good luck.
Dr. Moore: They really don’t care about the responses. They just want to follow the rules. And you’re going to as a cash practice doc, you’re going to have to have a broader spectrum of capability. If you can’t provide the service yourself, you need to know me or someone who can or Doctor Moore, who, by the way, I you know my limits.
I’m a good ultra sonographer. He says that I’m getting better or less good. Thank you. I’m getting better. But there are there are people who I work with who are the very best, like in the world. That’s who I get to work with. And because we reimburse them with cash, we tell them to say, how much does it cost to get this incredibly cutting edge therapy done? It’s $28,000. It’s like, oh, my gosh, who could possibly pay that? Well, if it’s life or death, if it’s survival, thriving and actually living well or being bedbound for the rest of your life, some people will happily come up with that much money. And it’s not for me to decide. Most of the work that we provide is 1500 bucks or less.
Sometimes. I mean, it doesn’t have to be that, but in the cash practice model, we are offering people something very different than the rest of the world. We’re offering them the opportunity to get the very best that’s available, regardless of insurance coverage.
There’s a lot of things that insurance covers. I’m super happy it does. But in the primary care world where we operate, primarily, this is the kind of thing that attracts patients who are going to tell their friends, it’s going to build your practice. You’re going to distinguish yourself as someone who not only is interested in getting people well, but who can.
So I really want to encourage you, this kind of thing where you develop a collegial relationship with people who can train you to be better at your work and train you in areas that are not available under the regular insurance model. Especially ultrasound is a steep curve to learn. It takes a long time to learn it, but to be good at it. But I think I got good enough at it within a few months to recognize the need for it.
Dr. Moore: And that’s a good point, and there is a learning curve to it. But if you start off with the basics, you know, it’s just how I teach people. Give me the foundation first and just work with that foundational level, and then you’ll grow, you know? But the whole thing with incorporating this type of service into a cash practice, it gives you a whole lot of freedom to offer these services to patients where they get a great response. They tell their friends, but you as a provider are getting a great deal of satisfaction. They’re like, wow, I’ve really helped that patient. And you actually probably feel a lot better about coming in the office everyday.
Dr. J: Yeah, I think that we talked about that, you know, converting your, the burnout to joy, you know, because of cash practice. It’s true. But a big part of that is also getting great outcomes.

Dr. Moore: I think it’s a really exciting. First quarter of 2026. And I’m going to launch a platelet rich plasma PRP fellowship program.
It’s a yearlong program where we meet, every other month. To, for an on site weekend where we are actually going to practice, procedures, on what, you know, either, volunteer patients or each other and the participants, and you will be evaluated on your ultrasound imaging skills and how you can put together a treatment plan. We’ll critique that. But then at the end of that one year program, you’re going to be one of the people, one of the few out there that actually has some skills and expertise instead of as somebody goes for a weekend seminar, it says, okay, now I’m going to come out and do regenerative medicine. Now you’re going to be, on a completely different level with that.
So commitment that, you know, is going to get you to a higher level.
Dr. J: Let me tell you about this guy. He wears me out every day. We see patients. He is so meticulous, so precise, highly skilled, which is that that’s the part doesn’t wear me out. That’s great. But, man, if you want to be trained and become great, he will not put up with, half-hearted, efforts. We and I think we’re the same that way with regard to the way that I provide patient care. If it’s not excellent, I don’t want to do it. At any insurance model, I can’t I’m to limit. I can’t do it. If you run a cash practice, you have the opportunity to provide full throated, absolutely top shot, top shelf care. The patient will pay you for the work that you do. You make an agreement with them that they’ll pay you for it. And it doesn’t have to be. You’re going to have crazy. Doctor Moore when he trains. He really wants you to be excellent so that when you’re done, you’re not going to have a piece of paper that says you can do stuff that you’re not any good at.
And the reason I love working with him is because I will tell you at the end of this listen, learn and love your patients.
I care about my patients deeply and I’m not going to turn them over to somebody who’s, mediocre. I’m just not going to do it so I can, without any hesitation, engage with Dr. Moore to say, hey, do this.
So if you’re interested in learning, real practical skills for ultrasound guided, injection therapy, especially in the regenerative field, he’s your guy. You should definitely look into it. And what? That’s a little bit higher level of skill and training, which I’m working with our staff to get our staff to that level as well, because we want to be so good that when a patient comes to us, we can say, we don’t know anybody better.
